Theres to much detail to explain and i cant really sum up in a few words how i did this.
All i can say is pen tool warp tool. Set layers to color dodge, screen, linear-dodge add.
The background is a radial background. There is a layer above the radial background which is a cloud render. Go to Filter render clouds. Set layer to color dodge and erase the area around focal with a soft brush set at 0% hardness and 100% opacity. This get the cloud/smoke effect.
